Who Qualifies?

Although Medicare does not require patients be homebound to cover physician house calls, a medical necessity and reason for the home visit (versus in-office) must be clearly documented for each encounter.

 

Optimal candidates for the SLO Home Medical Care MedicalHouse-Call Program are:

  • Enrolled in Medicare or one of the contracted Medicare Advantage Plans
  • Live in the SLO Home Medical Care service area – including zip codes 93401, 93402, 93405, 93407 and 93410
  • Have some degree of physical or mental (memory) limitation
  • Suffer chronic illness requiring regular physician oversight
  • Have difficulty otherwise accessing geriatric primary care services
  • Experience chronic or intermittent difficulty with mobility
  • Appreciate the attention to quality of life offered by a palliative-trained physician

Recognized benefits of Home Care Medicine include:

  • Ready access to medical care management of multiple chronic conditions
  • Reduced unnecessary ER visits and hospitalizations
  • Avoiding the infection risks of clinic and hospital settings
  • Improved involvement and education of home-based caregivers
  • Access to care during periods of limited mobility
  • A designated physician to provide oversight of all medical care
  • Improved patient and caregiver satisfaction
  • Decrease in institutional/nursing home placement
  • Improvement in knowledge of safety and provision of safe environment

At-home (house call) medical care is becoming the preferred setting or “first option” for many people, whether the focus is:

  • Self-management and preventive maintenance
  • Acute or chronic long-term care
  • Life-prolonging treatment
  • Short-term recuperation
  • Palliative care at life’s end
